- The Medical at Home Patient is defined as community dwelling adults or children having functional and/or medical impairments that prevent them from leaving their homes independently
- Medications are being delivered to this patient’s home out of complete necessity rather than convenience
- Receives assistance from an agency, family or other support service
- Patient has a caregiver (home health agency, family member, spouse, or nurse case manager)
- Pharmacy is compliance packaging the medication
- Provides a maximum 30-day supply
